   Geen fan van de Ridder maar Atir heeft serieuze concurrentie van Kalytera dat een middel heeft met veel betere resultaten. Voor alle duidelijkheid : ik zit in beide (Kiadis en Kalytera)

    Invoering
    Een ex vivo fotodepletiewerkwijze is ontwikkeld om ATIR101 (Kiadis Pharma) te produceren, een donorlymfocytinfusie toegediend na haploidentieke allogene hematopoëtische stamceltransplantatie (haplo-HSCT) om immuunreconstitutie te bevorderen. ATIR101 is ontdaan van alloreactieve T-cellen; vroege toediening na T-cel-uitgeputte haplo-HSCT heeft het potentieel om ernstige complicaties als gevolg van vertraagde immuunreconstitutie te verminderen. De veiligheid en werkzaamheid van ATIR101 worden gepresenteerd in een gepoolde analyse van 2 fase II-onderzoeken: CR-AIR-007, NCT01794299; CR-AIR-008, NCT02500550.
    
    methoden
    Een gepoolde analyse van 2 trials met vergelijkbare protocollen bij 37 volwassen patiënten (1 jaar follow-up in n = 34, data-cut-off 1 juni 2018). N = 32 patiënten kregen een enkele dosis ATIR101 (2,0 x 106 cellen / kg); n = 5 stopgezet na HSCT en ontving geen ATIR101 (overlijden n = 2, falen van het transplantaat n = 1, weigering van de partij n = 2). Van de 37 patiënten had 65% acute myeloïde leukemie, 27% had acute lymfoblastische leukemie en 8% had myelodysplastische syndromen; patiënten hadden een tussenliggende (57%) en een hoge (43%) ziekterisico-index. Allen ondergingen myeloablatieve conditionering gevolgd door een CD34 + -geselecteerde stamceltransplantatie van een haplo-identieke familiedonor. ATIR101, bereid uit dezelfde donor, werd toegediend op een mediaan van 28 dagen na HSCT zonder profylactische immunosuppressie. Uitkomsten werden vergeleken met een historische controlegroep van 35 patiënten behandeld met een T-cel-verarmd CD34 + geselecteerd haplo-HSCT zonder ATIR101 (CR-AIR-006, NCT02188290).
    
    resultaten
    Zesendertig patiënten waren geënt, met neutrofielen en bloedplaatjesinplanting op een mediaan (bereik) van 14 dagen (8-34) en 12 dagen (7-35) na respectievelijk HSCT. Niet-terugval mortaliteit (NRM) was 33% en terugval-gerelateerde mortaliteit was 8% vergeleken met 66% en 15%, respectievelijk, in de controlegroep (tabel). De totale overleving (OS) was 58% versus 20% in de controlegroep (figuur A). Totale chronische GVHD was 3% versus 11%, chronisch ernstige GVHD was 0% versus 9% en acute GVHD graad III-IV was 5% (alle graad III) versus 6% respectievelijk. Overlevingsanalyse zonder graad III / IV acute GVHD, chronische GVHD die systemische behandeling vereist, of terugval (GRFS) vertoonde significante scheiding voor de gepoolde cohort versus controlegroep (P = 0,01; Fig. B); 1-jaar GRFS-tarieven voor beide groepen waren respectievelijk 53% en 20%.
    
    Conclusie
    OS was ~3 keer hoger in de samengevoegde cohort dan in de controlegroep. Ziekte-terugval was beperkt en NRM in het samengevoegde cohort was de helft van dat van de controlegroep. ATIR101 werd goed verdragen. De resultaten suggereren dat ATIR101 de GVHD niet verhoogt boven de niveaus die zijn gerapporteerd met een T-cel-verarmd CD34 + geselecteerd HSCT alleen. GRFS was> 2,5 keer hoger dan in de controlegroep; de percentages lijken hoger dan gerapporteerd in studies met post-transplantatie cyclofosfamide na T-cel-verzadigd HSCT (Solh 2017, McCurdy 2017). Een fase III-studie van ATIR101 na T-cel-verarmd haplo-HSCT versus PTCy na T-cel-gevulde haplo-HSCT is aan de gang.
     
    
    Dit is de reden waarom het wordt goedgekeurd het geeft zeer zieke patiënten een betere overlevingskans. En ja DZR er gaan vreselijk genoeg dan nog steeds mensen dood maar de kans om te overleven is significant toegenomen.

    Six new medicines, including one cell-based therapy, recommended for approval
    
    The European Medicines Agency's Committee for Medicinal Products for Human Use (CHMP) recommended six new medicines, including one advanced therapy medicinal product (ATMP), for approval at its June meeting.
    
    The CHMP recommended granting a conditional marketing authorisation for the ATMP Zalmoxis as an adjunctive, or add-on, treatment for adult patients receiving a haploidentical haematopoietic stem cell transplant (HSCT) for types of blood cancer to aid immune reconstitution and reduce the risk of graft-versus-host disease. Zalmoxis has an orphan designation. For more information, please see the press release in the grid below.
    
    The Committee also recommended granting a marketing authorisation for Cinqaero (reslizumab) as an add-on treatment for adult patients with severe eosinophilic asthma.
    
    The generic medicine Atazanavir Mylan (atazanavir) was recommended by the CHMP for the treatment of human immunodeficiency virus-1 (HIV-1) infections.
    
    Three hybrid applications received positive recommendations from the Committee. Aerivio Spiromax and Airexar Spiromax, both containing salmeterol xinafoate and fluticasone propionate, were recommended for the treatment of asthma and chronic obstructive pulmonary disorder (COPD). Nordimet (methotrexate) was recommended for the treatment of active rheumatoid arthritis, juvenile idiopathic arthritis and severe psoriatic arthritis. Hybrid applications rely in part on the results of pre-clinical tests and clinical trials for a reference product and in part on new data.
    
    Request for re-examination of CHMP recommendation
    
    The applicant for Ninlaro has requested a re-examination of the CHMP's negative opinion for this medicine adopted at the May 2016 meeting. Upon receipt of the grounds for re-examination, the CHMP will re-examine this opinion and issue a final opinion.
    
    Seven recommendations on extensions of therapeutic indications
    
    The Committee recommended extensions of indications for Cervarix, Ilaris, Keytruda, Nevanac, RoActemra, Ryzodeg and Zontivity.
    
    Negative opinion on extension of indication
    
    The CHMP adopted a negative opinion on the request for an extension of therapeutic indication for Arzerra. A questions and answers document is available in the grid below.
    
    Start of review: Pharmaceutics International Inc
    
    The CHMP started a review of medicines manufactured by Pharmaceutics International Inc, USA. This follows an inspection in February 2016 which highlighted several shortcomings in relation to good manufacturing practice (GMP).     Outcome of Alkem Laboratories Ltd review
    
    The CHMP recommended the suspension of a medicine (Riluzole Alkem), for which studies were conducted at the Alkem Laboratories Ltd site in Taloja, India, and has required companies to provide new data for another medicine before it can be authorised in the EU.     Review of a safety signal for Adempas
    
    The Committee recommended that Adempas (riociguat) should not be used in patients with symptomatic pulmonary hypertension associated with idiopathic interstitial pneumonia or PH-IIP (high blood pressure in lung arteries caused by a lung disease called idiopathic interstitial pneumonia).     Update to product information for Noxafil
    
    The CHMP has warned that Noxafil (posaconazole) tablets and oral suspension have different doses and are not interchangeable.     Veel potentiele kopers aan zeer interessante koersen, maar weinig verkopers die met verlies willen verkopen. Dat zou ik nu toch niet direct als koersmanipulatie willen bestempelen.
